How to Submit a Complaint

To ensure a complete and accurate review, please fill out the complaint processing worksheet with as much detail as possible. Be sure to include your contact information so that we may follow up with you regarding your concerns. All complaints are taken seriously and will be reviewed promptly, thoroughly, and impartially.

At this time, complaint forms must be submitted in person or by U.S. mail. This requirement ensures that all complaints are properly documented, securely received, and verified for authenticity.
